How to Add Infant/Child CPR and Pediatric First Aid Documents

Important Guidelines to Follow:

If you have two separate documents — one for your certification and one for the training hours you completed to earn it:

• Upload your life-saving certification card to the Education section
• Upload your training hours certificate to the Training Received section

If your certification card also lists your training hours:

• Upload the same card to both the Education section and the Training Received section

1. In the Education Background section, click +Add Education Record

2. In the Type dropdown menu, select CPR, First Aid and MAT; in the Certification Type dropdown menu, select the appropriate life-saving option

3. Enter the required information, then choose I will upload a file for verification

4. Click the +File button to upload your life-saving documentation
   • For CPR and First Aid, cards must explicitly state that Infant/Child CPR or Pediatric First Aid is covered
   • Documentation must be a copy of the card or certificate issued from an approved institution

5. Click Save to proceed with the rest of your Education Background

6. Click Confirm to save your updates to the Education Background section, and continue building your profile.

You must Review and Confirm each section of your profile (even if you aren't uploading documents to that section) before you can Review Application and click Submit to notify us that your profile is ready for review. 

Click the attachment below to view a list of approved institutions. If you are mailing in your documents, you may submit photocopies of these items.
