Documents You May Be Asked to Submit

To complete your profile, we need documents to verify the information you have entered online. We encourage you to upload these documents directly to your profile, but you may also mail us photocopies of these items.  

 

Transcripts and Diplomas:

For all degrees or coursework listed in your profile, The Aspire Registry requires that you upload a copy of your official transcript from each college or university that you have attended.  You may also submit a copy of your diploma, but please note we can only verify your degree based on the information included in the diploma, and you will not be given credit for specific coursework.  This is particularly important if your program is a QUALITYstarsNY participant.  If you are mailing in your documents you may submit a photocopy of your transcript or degree.

Also, be sure to submit transcripts and diplomas for every degree you have earned, not just the highest degree you have completed, as this may impact your Career Ladder Level.

 

International Degree Evaluation:

For any international degrees you list on your profile, The Aspire Registry requires an evaluation of your transcript and diploma from an international credential evaluation institute, or an organization that is a member of the National Association of Credential Evaluation Services.

Teaching Certification

For any teaching certification you list on your profile, The Aspire Registry requires that you upload a copy of your teaching certificate issued by the NYS Department of Education, or the appropriate department in another state.  If you do not have a copy of your NYS teaching certificate to upload, you may also submit a screenshot of your TEACH account with the list of certifications you hold.  If you are mailing in your documents you may submit photocopies of these items.

Please note: The Aspire Registry cannot use certification exam results or certificates issued by your college or university to verify teaching certification. 

 

Child Development Associate and other Credentials

For any credential you list on your Aspire Profile, The Aspire Registry requires that you upload a copy of the actual credential issued by the awarding institution.  For instance, if you have earned your Child Development Associate Credential you would need to upload a copy of the credential you were given from the Council for Professional Recognition.  If you are mailing in your documents you may submit a photocopy of your credential.

Please note: The Aspire Registry cannot use coursework or transcripts to verify a Credential.

 

CPR, First Aid and MAT

For any CPR, First Aid or MAT certification listed on your profile, The Aspire Registry requires that you upload a copy of the card or certificate issued from an approved institution. The card or certificate must include the name of the institution, the type of certification(s), the date the training was conducted and the expiration date. To view a list of approved institutions, click here.  If you are mailing in your documents you may submit photocopies of these items.

Please note: If you would like the professional development hours for your certification captured on the training tab of your profile, the card must include the total number of training hours. If the card does not include the number of hours, we recommend that you submit both your card and the training certificate.

 

Training Documentation

Adding professional development to your profile is easy!  When you submit your documents for the first time, upload a copy of your training certificates for the past 2 years and our team will add them to your training tab. If you are mailing in your documents you may submit photocopies of your training certificates.

Don’t forget to keep your profile up-to-date! As you continue to attend professional development events throughout the year, capture this training in your profile by uploading new training certificates at any time.

Training certificates must include the following:

  • The name of the participant
  • The name and signature of the trainer or person who conducted the training event (This person cannot be the same person who received the training).
  • The name of the Training Organization
  • The title of the training
  • The date that the training took place
  • The total number of hours the training lasted

For accuracy, we recommend that the training certificate also include which Core Competency (CBK) and OCFS Training Priority Area(s) the training covers, and how much time is spent in each area. While The Aspire Registry will accept documentation without this information, Aspire staff will need to make a determination about the training topics each training covers, which may not be consistent with your experience or personal records.

Please note: The Aspire Registry requires training certificates and cannot accept agendas, invoices, receipts, quizzes/exams, or any handwritten documents to verify professional development or training hours.
